The following forms are for the 2010 UMYF Ski Trip. Please print out the appropriate registration form, and return it completed to the Church Office with any payment necessary (checks can be made out to UMYF).

If we do not have a recently completed health form (within the last year) from you on file, please print one out and bring it, completed, to the church office along with your registration.
